What “microdosing” actually means

There is no FDA-recognised microdose. The term describes doses below the lowest approved starting dose — sometimes as little as 0.05–0.25 mg of semaglutide weekly, against an approved starting dose of roughly 0.25 mg building toward a 2.4 mg maintenance dose for weight management. Wellness and telehealth companies define “microdose” arbitrarily, and definitions vary between providers — a “microdose” from one seller is not necessarily the same amount as from another.

What the evidence actually shows

This is the honest, unglamorous answer: we don’t have evidence that low-dose, intermittent use of GLP-1s provides meaningful or sustained weight loss. The trials that established these drugs work — the ones behind every figure on this site — tested the FDA-approved dose ranges, not fractional doses. Physicians quoted on the subject describe weight loss at very low doses as modest, in the region of 5–6%, compared with 15–20% at approved doses.

One partial exception: real-world data on the lowest approved semaglutide dose (1 mg, used for diabetes) found weight loss not far off the 2.4 mg weight-management dose in that specific comparison — a genuinely interesting finding that more is not always proportionally better within the approved range. That is a different question from microdosing below approved doses entirely, and it does not extend to the much lower amounts marketed as “microdoses.”

Absence of evidence is not evidence that it doesn’t work — but it is not evidence that it does, either. Right now, microdosing is an open question being marketed as a settled one.

Claims about anti-inflammatory, cardioprotective, or longevity benefits at microdose levels are, at this point, extrapolation from research on full-dose GLP-1 effects — not findings specific to very low doses. That may change as research continues; it is not where the evidence stands today.

The anti-inflammatory claim is the one that travels furthest, usually attached to an autoimmune condition, so it is worth knowing how much is being extrapolated from. Even at full approved doses the picture is thin and uneven: real randomised trials in plaque psoriasis, observational data only in inflammatory bowel disease, laboratory work alone in rheumatoid arthritis. Why a professional body has taken a position on this

The American Association of Clinical Endocrinology does not support routine microdosing of GLP-1 receptor agonists in clinical care, and recommends adherence to FDA-approved dosing. That is a notably direct position for a professional society to take, and reflects that the practice is happening in the market well ahead of any evidence base for it.

The specific risks worth knowing

  • Compounded microdose products lack FDA testing rigor and may have inconsistent actual dosing from batch to batch — the FDA has reported dosing errors with compounded GLP-1 products causing fainting and severe symptoms.
  • Because a microdose is usually accessed outside a full prescribing relationship for an approved indication, the same safety screening (thyroid history, pancreatitis history, pregnancy status) that applies to standard prescribing may not be applied as rigorously.
  • Self-directed dose-splitting or measuring — drawing a fraction of an injector-pen dose yourself — introduces real measurement error on a drug where the margin between doses matters.

Do microdoses have fewer side effects?

This is the most common reason people give for wanting one, and it deserves a careful answer rather than a reassuring one. The expectation is reasonable, and it has never been measured at these doses.

The reasoning behind it is sound as far as it goes. GLP-1 side effects are dose-related: the approved labels show nausea and the rest flaring after each dose increase and settling again, which is why every product is titrated upward slowly instead of started at the maintenance dose. A smaller dose should, on that logic, produce less of them. Nothing in the evidence contradicts that.

What does not follow is that anyone has checked. No trial has tested a sub-approved dose, so there is no adverse-event table for a microdose, no discontinuation rate, and no placebo arm to read them against — the figures on our side-effects pages all come from trials of approved doses, and none of them transfers. The same reasoning that predicts fewer side effects predicts less effect, and only one half of that trade tends to get quoted.

There is also a risk specific to microdosing rather than inherited from the drug. If the product is compounded and the dose is measured by drawing part of a pen or vial yourself, then the amount you receive is uncertain in a way an approved pen’s is not. In that situation “fewer side effects at a lower dose” assumes you know what dose you took, which is the assumption the whole practice rests on and the one nobody verifies.

What is actually being sold as a microdose

It is worth grounding this in something specific, because the word gets used loosely. Microdosing is not only a self-directed practice — it is a commercial product tier. Two things follow from that. The first is that a microdose bought this way is a compounded preparation, not an approved product, so the regulatory position is the compounding one rather than anything specific to the dose. The second is a matter of arithmetic that the marketing does not present. At $179 a month it costs more than the cheapest published price for an FDA-approved GLP-1, which is $149. A smaller dose is not automatically a cheaper one.
